To access and complete assessments, create an account and register as an individual, school, preschool, district, state/territory, or entity. Registration is free and simple! Once you are registered, visit the "Learn and Share SHAPE" tab for resources on how to use the SHAPE System and its tools.

School Mental Health Profile

The School Mental Health Profile (The Profile) is available to Individual, School, District, and Entity Accounts. The Profile collects information related to multi-tiered services and supports (MTSS), staffing, financing, and data systems in your school/district/entity school mental health systems.

The Profile:

  • has three sections and seven mixed-response-style questions (e.g., open ended, multiple choice, matrix)
  • takes about 20-40 minutes to complete.

School Mental Health Quality Assessment (SMH-QA)

The School Mental Health Quality Assessment (SMH-QA) is available to Individual, School, District, and Entity Accounts. The SMH-QA is designed for teams to access the comprehensiveness of their school mental health system and to identify priority areas for improvement.

The SMH-QA:

  • comprises seven domains
  • has between 70-98 questions depending on the version

Trauma-Responsive Schools Implementation Assessment (TRS-IA)

The Trauma-Responsive Schools Implementation Assessment (TRS-IA) is available to Individual, School, District, and Entity Accounts. The TRS-IA is an evidence-informed self-assessment quality improvement tool developed by the NCTSN Treatment and Services Adaptation Center for Resilience, Hope, and Wellness in School and the National Center for School Mental Health.

The TRS-IA:

  • comprises eight domains
  • has 32 questions
  • takes approximately 40-60 minutes to complete.

Organizational Well-being Inventory for Schools (OWBI-S)

The Organizational Well-Being Inventory for Schools (OWBI-S) is available to School and District Accounts. The OWBI-S assesses the quality of organizational well-being. Responses to the OWBI-S are submitted anonymously.

The OWBI-S:

  • comprises eight domains
  • has 52 questions
  • takes approximately 45 minutes to complete.

School Mental Health Family Engagement Quality Assessment - Coming Soon!

The School Mental Health Family Engagement Quality Assessment (SMHFE-QA) will soon be available to School and District Accounts. The SMHFE-QA will include two sections to assess family engagement and partnerships in your school mental health system. The first section focuses on the frequency schools engage in specific activities to partner with caregivers to promote positive mental health outcomes for students. The second section focuses on school’s ability to meet caregiver expectations for high quality school mental health system.

Culturally Responsive, Anti-Racist, and Equitable Schools Quality Assessment (CARES-QA)

The Culturally Responsive, Anti-Racist, and Equitable Schools Quality Assessment (CARES-QA) is available to School, District, and Entity Accounts and is designed to help team assessment of CARE practices and provides examples of ways districts can create an environment that reflects values around diversity, equity, and inclusion.

The CARES-QA:

  • comprises five domains
  • has 40 questions
  • takes approximately 15 minutes to complete.
